Globally, healthcare costs are expected to reach $10 trillion by 2024. The healthcare system suffers from a lack of continuity and integration in care delivery. Patients navigate through fragmented systems where their medical records, treatment histories, and care plans are scattered across different providers. This fragmentation leads to inefficiencies, redundant tests, and a lack of coordinated care, severely impacting patient experiences and outcomes. A study by the Public Health Foundation of India revealed that fragmented care is a significant contributor to poor health outcomes, particularly for chronic disease management.
