Patients navigate through fragmented systems where their medical records, treatment histories, and care plans are scattered across different providers. This fragmentation leads to inefficiencies, redundant tests, and a lack of coordinated care, severely impacting patient experiences and outcomes. A study by the Public Health Foundation of India revealed that fragmented care is a significant contributor to poor health outcomes, particularly for chronic disease management
